Air Canada near miss caught on video almost 'worst crash in aviation history

  AN AIR CANADA flight almost caused the “worst aviation crash in history” last year after being just feet away from hitting four other planes on the runway, according to the latest report. Safety officials say a near collision of airliners in  San Francisco  last year was a few feet from becoming the worst crash in aviation history and underscores the need for faster reporting of dangerous incidents before evidence is lost. The National Transportation Safety Board issued a final report Thursday on the incident in which an  Air Canada  jet nearly crashed into planes lined up on the ground at San Francisco International Airport. The pilots were slow to report the incident to superiors. By the time they did, the plane had made another flight and the cockpit voice recording of the close call was recorded over.  On 25 July 2000, Air France Flight 4590, a Concorde passenger jet on an international charter flight from Paris to New York, crashed shortly after takeoff, killing all 109 people on board and four on the ground. It was the only fatal Concorde accident during its 27-year operational history. The Concorde’s crash was caused by the loss of the two engines on the left side of the aircraft. A piece of debris on the runway caused one of the left side tires to disintegrate. When the tires exploded a piece hit the underside of the aircraft, which ruptured one of the fuel cells slightly ahead of the intakes to the engines 1 and 2. The fuel, which ignited, choked out the two engines on the left side, and the Concorde crashed into a hotel in Gonesse, France just 5 km from the runway.

  France Flight 8969 was an Air France flight that was hijacked on 24 December 1994 by the Armed Islamic Group of Algeria (GIA) at Houari Boumediene Airport, Algiers. The terrorists murdered three passengers and their intention was either to detonate the aircraft over the Eiffel Tower or the Tour Montparnasse in Paris. When the aircraft reached Marseille, the National Gendarmerie Intervention Group (GIGN), a counter-terror unit of the French National Gendarmerie, stormed the plane and killed all four hijackers.

 American Airlines Flight 191 was a tragic aviation accident that occurred on May 25, 1979. Shortly after taking off from O'Hare International Airport in Chicago, Illinois, headed for Los Angeles International Airport, the McDonnell Douglas DC-10-10 aircraft lost its left engine. This catastrophic engine detachment caused significant damage to the wing and its hydraulic and electrical systems. The plane rolled over and crashed into a field approximately half a mile away from the end of the runway, killing all 258 passengers and 13 crew members on board, as well as two people on the ground. It remains the deadliest aviation accident to have occurred in the United States. The investigation revealed maintenance and procedural failures, leading to significant changes in aviation safety practices and regulations.

On 29 April 2013, the Boeing 747-400 operating the flight crashed moments after taking off from Bagram, killing all seven people on board The subsequent investigation concluded that improperly secured cargo broke free during the take-off and rolled to the back of the cargo hold, crashing through the rear pressure bulkhead and disabling the rear flight control systems. This rendered the aircraft stuck in an uncontrollable pitch-up attitude and induced a stall, and made recovery by the pilots impossible.
